清化凉血化瘀方对急性肝衰竭大鼠肝细胞凋亡及Caspase-3、NF-κB表达的影响*

摘要:
目的:观察清化凉血化瘀方对急性肝衰竭大鼠肝细胞凋亡及胱天蛋白酶-3(Caspase-3)、核因子κB(NF-κB)表达的影响。方法:将160只大鼠随机分为中药组(清化凉血化瘀方灌胃)35只、西药组(复方甘草酸苷溶液灌胃)35只、模型组35只(灭菌蒸馏水灌胃)、中西医结合组(清化凉血化瘀方+复方甘草酸苷溶液灌胃)35只、空白组20只(灭菌蒸馏水灌胃),各组持续灌胃3 d后,空白组除外,按照药物剂量D-氨基半轧糖(D-Ga1)800 mg/kg、脂多糖(LPS)8 μg/kg,对剩余的140只各组大鼠制造急性肝衰竭大鼠的模型,行腹腔注射药物造模完成后,各组大鼠再次灌胃1次,分别给予相应的治疗药物溶液灌胃,24 h后行腹主动脉采血并处死各组大鼠,测各项生化及免疫指标。取大鼠肝组织病理标本,光镜下检测凋亡细胞;采用RT-PCR、免疫组化法、蛋白质印迹法检测大鼠肝组织Caspase-3、NF-κB表达水平的变化。结果:模型组大鼠肝组织见大量凋亡细胞,各治疗组凋亡细胞均显著减少,中西医结合组凋亡细胞减少最显著。与空白组比较,模型组大鼠肝组织Caspase-3 mRNA、NF-κB mRNA的表达明显增加,Caspase-3、NF-κB免疫组化指数明显升高,Caspase-3、NF-κB蛋白表达明显升高(P<0.05)。与模型组比较,各治疗组大鼠肝组织Caspase-3 mRNA、NF-κB mRNA的表达明显降低,Caspase-3、NF-κB免疫组化指数明显降低,Caspase-3、NF-κB蛋白表达明显降低(P<0.05),且以中西医结合组大鼠肝组织各项指标的下降程度最为显著,均低于中药组和西药组(P<0.05)。结论:清化凉血化瘀方联合西药的中西医结合治疗能下调急性肝衰竭大鼠Caspase-3、NF-κB的表达,有效减少肝细胞的凋亡,减轻肝细胞损伤。

Abstract:
Objective: To observe the effects of Qinghua Liangxue Huayu formula on hepatocyte apoptosis and the expressions of cysteinyl aspartate specific proteinase-3 (Caspase-3) and nuclear factor-κB (NF-κB) in rats with acute liver failure (ALF). Methods: A total of 160 rats were randomly divided into five groups, including the Chinese medicine (CM) group (gavaged with Qinghua Liangxue Huayu formula, n=35), the Western medicine (WM) group (gavaged with compound glycyrrhizin solution, n=35), the model group (gavaged with sterile distilled water, n=35), the integrated Chinese and Western medicine (ICWM) group (gavaged with Qinghua Liangxue Huayu formula and compound glycyrrhizin solution, n=35), and the blank group (gavaged with sterile distilled water, n=20). After 3 days of continuous gavage, except for the blank group, ALF models were induced in the remaining 140 rats by intraperitoneal injection of D-galactosamine (D-Gal, 800 mg/kg) and lipopolysaccharide (LPS, 8 μg/kg). Following model establishment, each group received another corresponding treatment via gavage. After 24 hours, blood was collected from the abdominal aorta and all rats were sacrificed for biochemical and immunological assays. Liver tissue samples were obtained for pathological examination. Apoptotic cells were observed under light microscopy after hematoxylin-eosin (HE) staining. The expression levels of Caspase-3 and NF-κB in liver tissues were detected by reverse transcription-polymerase chain reaction (RT-PCR), immunohistochemistry, and Western blotting, respectively. Results: Massive apoptotic cells were observed in the liver tissues of the model group, which were significantly reduced in all treatment groups, with the most pronounced reduction in the ICWM group. Compared with the blank group, the expressions of Caspase-3 mRNA, NF-κB mRNA, Caspase-3 protein, and NF-κB protein, as well as the immunohistochemical indices of Caspase-3 and NF-κB, were significantly increased in the model group (P<0.05). Compared with the model group, the expressions of Caspase-3 mRNA, NF-κB mRNA, Caspase-3 protein, and NF-κB protein, as well as the immunohistochemical indices of Caspase-3 and NF-κB, were significantly decreased in all treatment groups (P<0.05). Moreover, the decreases in all these indicators were most significant in the ICWM group, showing lower levels than those in the CM group and the WM group (P<0.05). Conclusion: The integrated treatment with Qinghua Liangxue Huayu gormula and Western medicine can down-regulate the expressions of Caspase-3 and NF-κB, effectively reduce hepatocyte apoptosis, and alleviate hepatocyte injury in rats with ALF.
